4.  UV Unwrap the low polygon model

Note:
For this example I used Unwrap (smart projections)

Low Poly model
Edit mode
Select all faces
U key
or
3D View menu >> Mesh >> UV Unwrap
UV calculation


  
5.  Map UV Texture

Note:  
For this example I used the UV Test Grid.

Add UV Test Grid
UV/Image Editor menu >> Image >> New
UV Test Grid
Give it a unique name
Press OK
   
Map unwrapped UV Texture to UV Test Grid
